Below you will find your credit card payment authorization form, sign and complete this form to authorize the reservation for someone else or if the credit card you held the reservation with will not be present at the time of the service.

By signing and sending this form you give us permission to debit your credit card, we will charge your credit card for the amount indicated on the reservation confirmation and/or fees involved such as parking, tolls and waiting time (if any) This authorization can be for a single or multiple transactions under the same account name, you can use this form to open a corporate or personal account such as frequent flyers without direct billing. Please be sure to include an image of the front's part of your credit card.
